So enough with my intro and here's the procedure and requirements to request for a Marriage Certificate.
First, go to your Local Civil Registrar located in your Municipal Hall or City Hall.
Present at least one valid ID. If it is not available, you may present your birth certificate.
Next is request for a Marriage Certificate Application Form from any issuing officer and fill in the following information:
- Name of husband;
- Maiden name of wife;
- Place of marriage;
- Date of marriage;
- Date of registration in case the marriage certificate was registered late;
- Purpose of your request; and
- Number of copies being requested;
If you're only a representative to get the marriage certificate, you must indicate your relationship with the owner of the certificate.
Next is pay the apllication fee of P330.00 per copy and wait for the issuing officer to process your request.
Lastly, affix your signature and done. Usually, the marriage certificate will be received two to three weeks after the wedding takes place.
So before you exchange vows with your future husband or wife, make sure you already have your marriage certificate processed.
