A birth certificate is one of the vital identity documents here in the Philippines. It identifies a complete personal information of an individual such as name, date and place of birth, residency, and parents.
Requirements/Procedures:
Go to your Local Civil Registrar (LCR) and ask where to request a birth certificate. Make sure you have at least one valid ID to verify your identity.
Listed below are considered valid IDs here in the Philippines:
- Company ID (make sure it has a signature of the employer or authorized representative);
- Postal ID;
- Students ID (you can use your library card if you have, signed by the school authority);
- Senior Citizens ID;
- Driver’s license;
- NBI Clearance
- PNP clearance;
- Passport;
- SSS ID;
- GSIS ID;
- Integrated Bar of the Philippine (IBP) ID;
- Professional Regulatory Commission (PRC) license.
Ask for a Birth Certificate Request Form from any issuing officer and fill in the following information:
-Name (if married female, you have to indicate the Maiden's name);
-Gender;
-Place of birth;
-Date of birth;
-Father’s name;
-Mother’s maiden name;
-Date of registration (if the birth certificate was registered late); and
- Indicate your purpose of requesting a birth certificate.
You need to specify as well the number of copies you want to request.
In case that it is not your birth certificate you are requesting or you're only a representative, you must indicate your relationship with the owner. I guess certain rules may apply.
An amount of P330 per copy must be paid.
The authorized officer will process your request. Once done, you may claim right away your birth certificate.
If you're applying for any goverment IDs/documents, you may present your birth certificate in case you don't have a valid ID.
